Vaginal moisturizer vs lube, and when you need both.

One is used on a schedule and changes your baseline. The other is used in the moment and changes the next hour. The difference, the numbers behind Replens and YES VM, and how to pick.

A vaginal moisturizer is used on a schedule, every two to three days, to keep tissue hydrated all the time. A lube is used in the moment to reduce friction during sex. If you are dry only during sex, you need a lube. If you are dry every day, you need a moisturizer, and probably a lube too.

The difference in one table.

Vaginal moisturizer Lube
When it is used On a schedule, whether or not sex is happening Right before and during sex
How often Every two to three days Each time, reapplied as needed
What it does Binds water to the vaginal tissue and releases it over days Reduces friction for roughly an hour
What it treats Daily dryness, itching, the tissue itself Friction during sex
How it is applied Applicator or insert, usually at bedtime By hand, on the body or a partner

The two get confused because they sit on the same shelf and both say "dryness" on the box. They are not interchangeable. People who buy a lube for daily dryness are disappointed because it is gone by lunch. People who buy a moisturizer for sex are disappointed because it was never meant for that.

What a moisturizer actually does.

A moisturizer is built around an ingredient that holds water. In Replens it is polycarbophil, a bioadhesive polymer that sticks to the vaginal wall and releases water as it slowly breaks down. In Revaree it is hyaluronic acid, which binds many times its weight in water. In YES VM it is a plant gum base with the same job. Applied at bedtime, the product stays put, and the tissue is better hydrated for the next two to three days.

That is why the Office on Women's Health lists moisturizers, not lube, as the first thing to try for menopause dryness. The lube comes second, for sex, if you still need it. Many women do.

The numbers, and why Replens reads differently.

We grade every lube on the lube index by osmolality and pH, using the WHO thresholds: 380 mOsm/kg or under is ideal, 1,200 is the ceiling, and a vaginal product should sit near pH 4.5. Moisturizers get the same treatment, with one wrinkle.

Product Osmolality (mOsm/kg) pH Used
YES VM 250 4.15 Every two to three days
Replens 1,200 to 2,011 2.5 to 3.5 Every three days
Revaree Not published Not published Every two to three days

YES VM is the clean case. It is iso-osmolar, its pH is inside the healthy range, and there is nothing to explain.

Replens is the interesting case. Its FDA filing shows osmolality at or above the WHO ceiling and a pH well below the healthy range. For a lube, those numbers would put it on our skip list. For a moisturizer the trade off is different. It is applied every three days, not during friction, and the low pH is part of the design: the product is meant to pull the vaginal environment back toward acidic after menopause lets it drift higher. Replens also has decades of clinical use and is the moisturizer most gynecologists name first.

The Edwards and Panay review in Climacteric argues that moisturizers, like lubes, should be as close to iso-osmolar and pH balanced as possible, and that many on the market are not. We agree with the principle. So the practical read is this: if your tissue is thin, irritated or easily stung, start with YES VM, whose numbers are closest to the body. If your clinician has you on Replens and it is working, that is a fine answer too.

Revaree publishes neither number. Hyaluronic acid inserts have a good reputation and a format people stick with, but we cannot grade what is not published.

When you need both.

The common case after menopause, while breastfeeding, or after some cancer treatments is daily dryness plus sex that has started to hurt. That is a two product problem. The moisturizer, used consistently for two to three weeks, changes the baseline. The lube, used during sex, handles the rest. A low osmolality water based lube like Good Clean Love Almost Naked (240 to 269 mOsm/kg, pH 4.7) pairs with any of the moisturizers above. If dryness is significant, a silicone lube lasts longer, and we cover that in the menopause pick.

Two things a moisturizer cannot do.

Replace a lube. Most moisturizers are not designed for friction. Some brands say theirs can double as a lube, but Replens in particular is meant to be applied well ahead of sex, and a product built to stay put for three days is not a product built to be slippery for an hour. Keep both.

Replace a clinician. Moisturizers treat the symptom. They do not restore thinned tissue. If you have used one consistently for a month and sex still hurts, or you have burning, bleeding or urinary symptoms, see a clinician. Low dose vaginal estrogen and other prescription options exist for exactly this gap, and they are a routine conversation, not a last resort.

Questions people ask.

What is the difference between a vaginal moisturizer and a lubricant?
A moisturizer is used on a schedule, every two to three days, to keep the tissue hydrated all the time. A lube is used right before sex to make it slippery for the next hour. One treats the dryness; the other treats the friction.
Can I use lube as a vaginal moisturizer?
Not really. Water based lube evaporates or absorbs within an hour or two, so it does nothing for tissue at the other end of the day. Moisturizers are built to bind water and hold it for days.
Can I use a vaginal moisturizer as lube?
Some people do, and a few brands say it is fine, but most moisturizers are not designed for friction and some (Replens, for example) are meant to be used well before sex, not during. Keep a separate lube.
Which vaginal moisturizer do gynecologists recommend?
Replens is the one named most often, because it has been around the longest. YES VM has the best published osmolality and pH. Hyaluronic acid inserts like Revaree are the newer option people tend to stick with.
How long does a vaginal moisturizer take to work?
Give it two to three weeks of consistent use. If daily dryness has not improved by then, see a clinician about prescription options.
